Form GC-335: Capacity Declaration-Conservatorship

Form GC-335 is completed by a physician or qualified psychologist describing the proposed conservatee’s capacity. It is required when the petition asks for medical consent powers, or when the conservatee cannot attend the hearing.
Common Mistakes
The doctor must complete it fully and recently – stale or partially completed declarations draw examiner notes and can push your hearing date.
