Form GC-312: Confidential Supplemental Information (Probate Conservatorship)

Form GC-312 is the private companion to the petition: health details, living situation, and why alternatives to conservatorship (powers of attorney, supported decision-making) are not sufficient. “Confidential” means only the court and specified parties see it.

Common Mistakes

The alternatives section is where inexperienced filers stumble – the court expects a genuine explanation of what was considered and why it will not work, not “N/A.”
