Form GC-310 is the starting document of every probate conservatorship: it asks the court to appoint a conservator, identifies the proposed conservatee and petitioner, states which powers are requested (person, estate, or both), and explains why the conservatorship is needed.

The person seeking appointment as conservator – or any relative or friend petitioning on behalf of a proposed conservator – files GC-310 to open the case in the probate division.

Common Mistakes

Leaving the “powers requested” attachments incomplete, and mismatching names across the packet – the court examiner checks every page against this petition. GC-310 is filed together with GC-312, GC-314, GC-320, and GC-020.
