Form GC-079: Pre-Move Notice of Proposed Change of Personal Residence

Form GC-079 is filed by an existing conservator planning to move the conservatee to a new residence: the court and relatives get advance notice – generally 15 days – before the move happens.

Important Limit

Out-of-state moves need more than this notice; they require court permission. If you are at this stage, the conservatorship already exists – we prepare the notice and the proofs of service that go with it.
